“Measured” figures use the average block spacing over the last 60 indexed blocks; “target” figures assume every block lands exactly on the protocol's thirty-second target, which it never does exactly. The difficulty retarget window is 60 blocks; the measured timespan of that window is clamped to between 885 and 3,540 seconds before the next target is derived, so difficulty can change by at most a factor of two, up or down, per window.
